    Physical Fitness Data Collection Template

    in Elementary PE Idea Tank

The state of Illinois requires fitness data to be submitted to the state. For that purpose, the ISBE created an Excel Spreadsheet. To make this useful to G-Suite Districts, I have converted it to Google Sheets here: goo.gl/gEWvHE This first tab includes the directions, with the data sheets labeled "Scores(G3)", "Scores(G4)", etc.

The spreadsheet automatically determines Healthy Fitness Zone (or not) for Aerobic Capacity (Pacer), Flexibility (Back Saver Sit and Reach), Muscular Endurance (Curl Up), Muscular Strength (90 Degree Push Up).

It, also, does cumulative counts for Males and Females, Healthy Fitness Zone and Needs Improvement, and Total Counts for each.
