Adam has done a terrific job of explaining his use of Form Publisher here: goo.gl/5MqpUd to evaluate student skill performance.
He uses these two files:
Analysis Report Template: goo.gl/mScFwt (This is the template that Form Publisher uses to create each student file.)
Analysis of Performance Google Form: goo.gl/spn1nN (This is the form used by the evaluator.)
The way it works is that the person doing the evaluation, simply opens the form and enters the required information including the specific questions evaluating the skill being tested. I love Adam's use of images and coaching points to cue the evaluator. Once the evaluation has been submitted, immediately and automatically, the file will be generated and sent to the participant (the student being evaluated) and to you, as the teacher. (When setting up Form Publisher, you will want to add <<Participant Email>> as a collaborator so that they will receive an email with the evaluation attached.)

The great thing about this is that by simply making a copy of the google form and editing it per skill, you can use this method for peer evaluation as well as teacher evaluation of ANY skill.
